Upload Your Documents

Please use the form below to upload your supporting documents.  The form will accept compressed “zip” files to a maximum of 10MB (megabytes).

To create a zip file:

1. Create a new folder on your desktop (right-click > New > Folder).
2. Add your supporting documents into that new folder.
3. Right-click on that new folder: Send to > Compressed (zipped) folder
